Less severe heart failure
A definite diagnosis by a consultant cardiologist of failure of the heart to function as a pump. There must be evidence of the following:
- permanent and irreversible limitation of function to at least Class III of the New York Heart Association classification; and
- permanent and irreversible ejection fraction of more than 39% and less than 50%.
